The Doctors are not able to provide a fit note (previously known as a sick note) unless you have been off work for more than 7 days in a row including non-working days.
Self Certification
Your employer can ask you to fill in a form when you return to work to confirm that you have been off work sick for up to 7days.This is called self certification.They can supply you with this or you can download from https://www.hmrc.gov.uk/forms/sc2.pdf
